This includes implementing remote monitoring operations and/or vendor selection that will best suit the practice; optimizing billing and compliance and ensuring seamless patient connectivity and in-office checks and follow ups.
- Chronic Care Management (CCM)
- Developing and executing comprehensive care plans for patients with multiple chronic conditions, enhancing patient engagement, improving health outcomes, and maximizing reimbursement opportunities. Services include service line development and/or vendor selection, patient enrollment, care coordination, documentation optimization, and compliance with Medicare guidelines.
- Principle Care Management (PCM)
- Establishing structured care management for patients with a single high-risk chronic condition that requires intensive monitoring and treatment. This includes program setup, patient tracking, provider coordination, and reimbursement optimization to ensure effective care delivery
- Remote Physiologic Management (RPM)
- Implementing and managing remote monitoring programs that track patient vitals—such as blood pressure, glucose levels, oxygen saturation, and weight—via connected medical devices. This includes selecting the right technology, integrating it with EHR systems, ensuring billing compliance, and training staff for patient engagement.
- Medical Weight Loss Programs
- Designing and executing physician-supervised weight loss programs, incorporating GLP-1 medications. Services include program structuring, financial modeling, patient adherence strategies, and FDA compliance to maximize clinical and financial outcomes.
